Statutory declaration

If you are ordering a document by mail, you must provide your ID through a statutory declaration.

What’s important to know

The application form and statutory declaration must be originals.

Don’t attach a copy of your identification to the application.

If you are outside of Alberta and are applying by mail, your ID is provided by using the statutory declaration on the Registry Connect application form below.

  1. Complete the application

    Application for Vital Statistics Documents by Mail (PDF, 8 pages)

    Note: To download a PDF document, right-click and select 'save as' and save to your computer.

  2. Get the application notarized

    Take your acceptable ID and application to a Commissioner for Oaths or Notary Public to complete the Statutory Declaration for Proof of Identity section of the application form.

  3. Send the statutory declaration

    Send the statutory declaration with the original application for Proof of Identity to Registry Connect.
